Important Notice - Effective October 1, 2025

This Annual Facility Maintenance Permit is valid for 12 months and covers routine maintenance, emergency repairs, and minor renovations. Individual projects must not exceed $200,000 per Florida Statute �553.80(6).

Important Cost Limitation

Per Florida Statute �553.80(6), individual projects under this permit cannot exceed $200,000. Projects exceeding this amount require separate permitting and comprehensive plan review.

Compliance Acknowledgments

I acknowledge that all work must be documented in a centralized log including work description, location, date, personnel, and inspection outcomes *

I understand that mandatory inspections are required for fire protection systems, hot work, and electrical upgrades using BCA-Form 05 *

I commit to submitting quarterly reports (minimum) to the Building Code Administrator detailing completed work and inspections *

I confirm that maintenance staff and project managers will receive training on permit scope, documentation, and code compliance *

I understand that failure to maintain proper documentation will result in permit revocation and permanent restriction from annual permits *

Exclusions Acknowledgment

Work NOT Covered by This Permit

This permit explicitly excludes: structural changes, occupancy modifications, major system overhauls, and any project exceeding $200,000. Such work requires separate permitting.
